Thank you for your interest in joining us at Nambour Heights Family Medical!
Please read our fees page before booking online, as we are a private billing practice for all those above 24 months of age and without a DVA Gold Card. The fee for the appointment depends on the length of your consultation with the doctor, a standard appointment is 12 minutes. If booking with Dr Eagles he will generally charge a long appointment fee for the first consult. Dr Eagles will not bulk bill children under 24 months, only those with a DVA gold card.
We will process your Medicare rebate for you after every appointment, provided you can supply a valid Medicare card. We do not charge an Eftpos surcharge.
Please be aware that as a new patient your first appointment should be booked as long, in order for our doctors to provide you the best level of care. You should arrive at least 10 minutes early for your initial appointment to give yourself time to fill in the new patient forms, if you have not already filled them out here and returned them to our reception staff at nambourheights@familymedicalgp.com.au.
Appointments for requesting ADHD, GLP-1 or HRT medications must be booked as long to provide your doctor adequate time to assess your request.
You may be asked to provide your consent to request information from your previous surgery, and you are more than welcome to request this yourself with our reception staff, who will assist in getting your information sent to your new doctor at our practice.
Please do not book health assessments, driving medicals, vaccinations, ECGs, iron infusions, ear syringing or new baby checks online, as we need to allocate nursing staff time for the appointment. Please call our reception staff on 07 5441 6093 if unsure.
